Tips for Using the Personal Health Record Feature

  • Make a new account by clicking “New User” or “Register Now”.
  • You will be directed to a screen titled “Participate: Set up a New Account”.
  • Enter your personal information in the boxes provided, review the user agreement, and the click “submit” to set up your new account.
  • Once you have created your account you will be able to view your Personal Health Record.
  • On the left side of your screen, you will see a blue button “>> Get Started Now
    • By clicking this button, you can add and manage folders containing health record information
    • Under these folders, you will find Add Contact Information where you can name the folder and input first and last names with their associated contact information
    • Under Add Personal Information and Add Lifestyle Information, input the information you know and have access to.
    • In addition, ask your doctor if you need help making your personal health record.
  • In the center of your screen, you will see the messages you have received in the past 7 days.
  • On the top right side of your screen, you will see the buttons “Logout, My Account, My Sub-Folders, and PHR Home” to help you navigate the website better.
  • For more information or any questions you have about your Personal Health Record, click on the green icon titled “LEARN MORE Personal Health Records” on the left side of the home page.
